WAIVER

PARTICIPANT AGREEMENT + LIABILITY WAIVER

I, the undersigned parent or legal guardian, understand and acknowledge the following:

1. Voluntary Participation: I am voluntarily allowing my child to participate in physical activities including but not limited to aerial arts, tumbling, games, yoga, and general movement at The Fix Aerial Arts and/or Infinite Athletics.

2. Assumption of Risk: I understand these activities involve physical exertion and carry inherent risks including falls, sprains, bruises, and other injuries. I assume full responsibility for my child’s participation and waive any and all claims against The Fix Aerial Arts, Infinite Athletics, their owners, instructors, coaches, and staff.

3. Medical Acknowledgment: I affirm my child is in good health and able to safely participate. I have disclosed all relevant medical concerns, allergies, or behavioral needs.

4. Emergency Care Authorization: In case of illness or injury, I authorize staff from either organization to administer first aid and, if necessary, secure medical treatment on my child’s behalf.

5. Photo + Video Release (Optional): I grant permission for The Fix Aerial Arts and/or Infinite Athletics to use photos or video of my child taken during the event for promotional purposes. (Option to opt out below.)

6. Property Disclaimer: I understand The Fix and Infinite Athletics are not responsible for lost, stolen, or damaged personal belongings.
